WHAT DO I NEED TO DO?

Bring a current photo ID for all adults 18 and older.

For children under the age of 18, you must provide one of the following:  

*Birth Certificate  *Social Security Card  *Passport   *Legal Documents   *Medicaid card with the individual's name

*School Social Worker letter stating the child is in the household

 

RENT:  Bring your eviction notice and/or a letter from your landlord.

The letter must include:  your address, the amount of late rent, amount of late fees, the date due, your landlord's address and phone number.

If your landlord has not already submitted a W-9 to us, we would also need that before we can provide any assistance for rent.

 

UTILITIES:  Bring your disconnect notice AND your most recent bill.

Please come in as soon as you receive the disconnect notice as it takes time to process this with the utility company.

 

MEDICATION ASSISTANCE:  Bring a printout from your pharmacy stating what medications you need and the amount for each one.

Please note:  WE DO NOT ASSIST WITH ANY CONTROLLED MEDICATIONS or SELECTED OTHER CLASSES OF PRESCRIPTION DRUGS.

 

WHAT HAPPENS NEXT?

When you come in, you will complete an application with one of our intake coordinators.  Be ready to discuss 30 days of income and 30 days of expenses.

You will be asked to call the Kendallville office at 260-349-1942 on the following business day to hear whether or not we are able to assist.

If we are able to assist, you will be given the conditions of that assistance.  For example, you might have to pay a part of the bill before we can pay our portion.

Once you have worked out your part of the conditions, you will return to the Kendallvlle office to receive the remainder of your assistance.
